If you are a tobacco user and want to quit, this program is for you.
The North Dakota Public Employees Retirement System received a grant from North Dakota Department of Health to help state employees and their dependents age 18 and older quit smoking or chewing tobacco. The grant will pay for approved counseling, medications and health care provider visits and co-pays. This project is administered by Blue Cross Blue Shield of North Dakota.
Who is eligible?
All current state employees and their dependents age 18 and older, have NDPERS health coverage, who smoke or chew tobacco and want to quit are eligible to participate. County, city and other members of the NDPERS group are not eligible through this project; however, smoking cessation funds may be available at the local level.
How do I get started?
You may start anytime during the enrollment period July 1 2013. Benefits will be paid until the grant dollars are gone.
What will be provided?
The program allows for telephone counseling through NDQuits phone, mobile, or online cessation services, or pre-approved face-to-face tobacco cessation counseling. The program pays up to $700 in expenses, including:
- Up to $200 for your office visit and co-pays. (If face-to-face counseling is approved, this will be included in the $200 maximum for office visits).
- Up to $500 every six months for the following FDA-approved medications:
- Over-the-counter: nicotine gum, nicotine patch, nicotine lozenge
- Prescription: Bupropion, Chantix, nicotine nasal spray and nicotine inhaler
How do I enroll?
1.) Contact Blue Cross Blue Shield of North Dakota at 1.800.223.1704 or 701.282.1400 if calling from the Fargo area. Blue Cross Blue Shield will verify your eligibility for this program and will send a cessation program identification card to use when you visit your healthcare provider.
2.) Contact NDQuits Phone at 1.800.QUIT.NOW (1.800.784.8669) or log on to NDQuits Online at nd.quitlogix.org to enroll in counseling. Members interested in face-to-face counseling must contact the North Dakota Department of Health at 1.866.280.5512 for pre-approval. Face- to-face counseling is eligible for financial reimbursement up to $200.
3.) Members choosing to use FDA approved cessation medications are eligible for financial reimbursement up to $500 every six months for the following FDA approved medications:
Over the Counter: nicotine gum, nicotine patch, nicotine lozenge.
Prescription: Bupropion, Chantix, nicotine nasal spray and nicotine inhaler.
Prescription and over-the-counter medications will require payment at the pharmacy. Keep your receipts and send them to Blue Cross Blue Shield of North Dakota to the address shown on the back of your cessation program identification card.
4.) Be sure to show your cessation program identification card to your healthcare professional
Your healthcare provider should submit the office visit claim to the address shown on the back of the cessation program identification card.
