Blue Cross Blue Shield of North Dakota (BCBSND) is partnering with Arkos Health to expand our existing care management capabilities and elevate well-being in North Dakota. The new partnership and enhanced services will launch in the fall.
Arkos is a population health management provider that works collaboratively with health insurers and a member’s primary care team to help provide supplemental coordinated care to Medicaid Expansion members at no additional cost. Examples of how Arkos may support Medicaid Expansion include:
- Engaging with members after a hospital stay by ensuring they have filled their prescribed medications and made necessary follow-up visits with their primary care provider.
- Helping members know where to go for care.
- Assessing and coordinating community resources.
- Helping patients comply with their prescribed treatment plan.
This added support is all about meeting members where they’re at and closing gaps where they may otherwise fall through the cracks.
