Health center superheroes use MediQHome to help patients

A group of superheroes have helped Robin Dyke improve her health. No, really.

Coal Country Community Health superheroesThe super-human doctors and staff members at Coal Country Community Health Center in western North Dakota are helped Dyke and other patients fight diabetes and other chronic conditions through their participation in our MediQHome program. After of her diabetes diagnosis, Dyke has been working with doctors who are coordinating her care. As a result, she is now living a healthier lifestyle and her health has improved.

MediQHome allows doctors to serve as a medical home for patients with chronic conditions such as diabetes or high blood pressure — using a technology platform to help their doctor coordinate tests, appointments with specialists and procedures, ensuring they receive needed screenings and the right care at the right time.

"Our patients are receiving better care and are more knowledgeable about the care they are receiving," said Aaron Garman, a doctor at Coal Country Community Health Center.

Coal Country Community Health Center, with locations in Beulah, Center and Towner, received the Best Rural Health Performer for Optimal Diabetes Care award at our Innovations in Health Care Awards last fall for its use of MediQHome to improve patient care. Watch out diabetes, they're coming for you!
