At Blue Cross Blue Shield of North Dakota (BCBSND), we want all members to have high-quality and affordable health care that supports their well-being. To keep that commitment, we are changing how we cover weight-loss drugs.
Why We’re Making Changes
GLP-1 weight-loss drugs, such as Wegovy, Saxenda and Zepbound, have gained attention and use across the nation to help people lose weight. While these drugs can be effective for some people, they also come with big challenges, including cost and consistent long-term use. In 2025, spending on these drugs increased by 46%. And we’re estimating that total costs can reach $23 million for commercial plans.
Studies also raised important questions about the long-term value and sustainability, particularly when these drugs can cost more than $1,000 per month. Recent studies showed that while adherence to these drugs is improving, people tend to stop their treatment too soon. According to Prime Therapeutics, only 8% to 14% of people stayed on their treatment plan after three years*.
These costs and trends have a big impact on the people we serve. That’s why we’re taking a thoughtful approach to try to make health care more affordable for all our members.
What’s Changing in 2026
Here’s a breakdown of the new changes starting Jan. 1, 2026 upon plan anniversary:
- Fully Insured Large Group Plans (51+ contracts):
Coverage for weight-loss drugs, including GLP-1s and oral medications, will be removed. This change is meant to help keep premiums stable and encourage drug makers to find ways to improve prices. - Self-Funded Plans:
Clients will need to choose to include these weight-loss drugs with their plan. They also have the option to add Vida Health to support members.
Why Lifestyle Management Matters
Weight-loss drugs work best when combined with lifestyle changes. Programs like Vida Health and Omada offer coaching, digital tools and wellness support to help members develop healthy habits. Pairing these drugs and programs helps members lose weight in a way that lasts. It helps them stick to a treatment plan. It also lowers the risk of other health issues.
This approach helps improve their health while making care more affordable for everyone.
