Nominations for three $5,000 awards are now open through Tuesday, Oct. 31
FARGO, N.D. Blue Cross Blue Shield of North Dakota (BCBSND) Caring Foundation is taking nominations for its new Community Champion Awards program. These awards recognize high-impact community partners working to improve health equity, fill gaps in health care and ensure health care access in their communities. Overall, the program will recognize three organizations who will each receive $5,000.
Nomination details
To apply, applicants must:
- Be a community-based or civic organization, non-profit or foundation, health care or social service provider
- Demonstrate measurable improvements in health disparities through data-driven initiatives that address social determinants of health (SDoH) and disparities within underserved populations
- Create models and inspire other organizations to initiate community-centered solutions to improve health and well-being
Applications must be submitted by Tuesday, Oct. 31, 2023 at 5 p.m. (CST) to the BCBSND Caring Foundation via email at caringfoundation@bcbsnd.com.

The Community Champion Awards were inspired by the findings of the Caring Foundation’s study on the Social Determinants of Health in partnership with the Center for Social Research at North Dakota State University. The study reveals several persistent economic and social disparities in North Dakota that require community driven solutions to address.
