Information required for small employer group quotes
Please have the following information ready before completing a quote for a small employer group:
- Client name and address
- Effectve date
- SIC code
- Total number of benefit eligible employees
- Following details for all eligible employees (even if waiving coverage, if possible)
- Full name
- Gender
- Date of birth of all employees and family members
