During open enrollment, applications for a 1/1 effective date should be completed online. For on exchange plans, Sign in to HealthSherpa to enroll a client. To get a quote or enroll a client off exchange, use the quote to enrollment form. Paper applications will not be processed.

For a first of the month effective date, the application must be received by BCBSND prior to the first (the requested effective date).
 

i.e. If a member wants a plan starting Feb. 1, the application must be received by Jan. 31.

All the individuals applying for insurance must be noted on the COC as having lost insurance.
 

A COC is needed if the qualifying event is loss of employer group coverage, permanent move affecting eligibility, loss of COBRA, loss of Medicaid, etc.

Yes, include SSNs on all applications (exception for newborns who don’t yet have a SSN).
