NextBlue is informing providers of a new step therapy requirement to be added for Pemetrexed drugs for dates of service on or after April 26, 2024, members must try and fail two other pemetrexed drugs before we will approve prior authorization requests for Pemfexy® (pemetrexed), HCPCS code J9304.
