Healthcare Effectiveness Data and Information Set Medical Record Requests

Blue Cross Blue Shield of North Dakota (BCBSND) is responsible for the accuracy and integrity of the Healthcare Effectiveness Data and Information Set (HEDIS) measure documentation. As part of our commitment to ensuring the accuracy and reliability of HEDIS measure data, we are conducting a primary source verification (PSV) audit of the supplemental data that has been submitted.

The purpose of this request is to verify that the supplemental data we have received matches the original source documentation. This verification process is essential for maintaining the integrity of our data and ensuring compliance with National Committee for Quality Assurance (NCQA) standards.

To support this effort, we are requesting the health records for the patients in a medical records request letter. If your facility receives the request letter, please return to the attention of the BCBSND Quality Management Department with the requested medical record within five business days of this request.

Providers are permitted to disclose protected health information (PHI) to health plans without authorization from the patient when both the provider and the health plan have a relationship with the patient and information related to the relationship.
