Revision-Notice of Change for Discharge Summary Notification

Blue Cross Blue Shield of North Dakota (BCBSND) wants to collaborate with our providers on post discharge care for Medicaid Expansion members in the following settings:

  • Inpatient rehab
  • Inpatient hospital
  • Long term acute care
  • Skilled nursing facilities/transitional care units
  • Residential treatment centers

Our care teams offer additional follow-up care to these members who are being discharged. Follow-up calls to Medicaid Expansion members are attempted within seven (7) days of discharge. Our goal, much like the health systems, is to help reduce re-admission and assist in the care need for post discharge. With provider collaboration, we can improve adherence to the established treatment plan from their recent stay.

To assist our care team with this outreach, effective July 7, 2025, we are asking the identified provider facilities (listed above) to submit the information below via fax to 701-277-2971:

  • The prior authorization number of the approved stay, when applicable
  • Member name, date of birth and BCBSND member ID (UMI)
  • Discharge date, diagnosis, medications/prescriptions
  • Discharge notes, including any after-care appointments (date, time and who will follow up) and indication the patient understands the care plan.

This information will be placed in the next update to the provider manual, which we expect to happen in quarter four of this year.
