Updates to 2026 Weight Loss Medication Coverage

What Providers Need to Know

Blue Cross Blue Shield of North Dakota (BCBSND) is updating how its commercial plans cover weight-loss medications to manage rising costs while continuing to support members with sustainable, high-quality care. Spending on weight-loss GLP-1s increased 46% in 2025 and is protected to reach a total cost of $23 million. While these medications can be effective, long-term adherence remains low and costs are impacting premiums across the board. 

To address these trends, the following changes go into effect on Jan. 1, 2026, and upon plan anniversary:

  • Fully insured large group plans will no longer cover weight-loss medications.
  • Self-funded clients must choose to cover weight-loss medications and may add Vida Health as a buy-up for holistic support.
    • For self-funded clients who choose Vida Health, a member will have to get their weight loss GLP-1s from a Vida provider.
    • This means if you are prescribed a weight-loss GLP-1, you will need to receive your prescription and ongoing management directly through a Vida Health provider, rather than your current care team.
    • This change isn’t a reflection on the quality of care being delivered, as many providers have been directly managing their patients’ weight loss.
    • This is a strategic effort to ensure consistency, improve adherence and manage the high cost of these medications.
