Starting January 2025, eligible fully insured commercial members will be mailed letters notifying them of eligibility to receive comprehensive medication management (CMM) services from a pharmacist provider. Participation in CMM services is optional for BCBSND members.
CMM services provided to BCBSND members must be documented through the Outcomes.com patient-centric web-based platform to receive payment for encounters. Any licensed pharmacist contracted with Outcomes can create a new user account to provide CMM services for eligible members at Outcomes.com. Pharmacist providers should be reaching out to members’ medication prescribers to collaborate concerning CMM services.
