The following medical policy was reviewed by our Medical Policy Team and has coding changes. This policy change will be effective on Feb. 1, 2026, and updated on our website at:

- Added 92972, C9764, C9765, C9766, C9767, C9772, C9773, C9774, and C9775 were inadvertently removed from the policy back in September 2025
- E0743 and A4544 were inadvertently removed from the policy back in July 2025
- Added January 2026 New Code 63032
