New Form Available for Providers- Medical Policy Development, Adoption and Reviewing Form

Blue Cross Blue Shield of North Dakota (BCBSND) has a new form available.

This form allows practitioners with relevant clinical expertise to provide input and feedback on the development, adoption and application of Utilization Management (UM) criteria for medical policy. It is located on the www.bcbsnd.com Forms and Documents page here.

Please note the importance of filling the form out in its entirety and attaching all relevant supporting documentation before submitting it to BCBSND.
