Access to the drug formulary lists for April 2025 are located at Provider Pharmacy Information. Our member community can also access to the drug formulary lists at Pharmacy Tools.
These lists provide information on covered medications, pharmaceutical management procedures, prior approval and exceptions processes, dispensing limits and more.
