Blue Cross Blue Shield of North Dakota (BCBSND) wants to remind providers that modifier AF is required for Office or Outpatient E/M codes 99202 – 99215 when billed with a primary Behavioral Health diagnosis code to receive the Behavioral Health fee schedule amount.
Medicaid Expansion claims billed with E/M codes 99202 – 99215 with Modifier AF without a primary behavioral health diagnosis will be rejected as non-payable, which is a provider liable rejection. This will become effective for claims processed on or after Aug. 17, 2026.
