Description

Polymerized sucralfate malate paste (ProThelial^TM) forms a protective layer over the oral mucosa by adhering to the mucosal surface. The paste may be used in the management of mouth lesions of all types including aphthous ulcer, stomatitis, mucositis, minor lesions, chafing and traumatic ulcers, abrasions caused by braces and ill-fitting dentures, and lesions associated with oral surgery.

Criteria

Coverage is subject to the specific terms of the member’s benefit plan.

Federal Employee Program members (FEP) should check with their Retail Pharmacy Program to determine if prior approval is required by calling the Retail Pharmacy Program at 1-800-624-5060 (TTY: 1-800-624-5077). FEP members can also obtain the list through the www.fepblue.org website.

Polymerized Sucralfate Malate Paste (ProThelial) is considered experimental/investigational and therefore non-covered as there is a lack of conclusive evidence confirming clinical efficacy.
