Childhood Immunization Status (CIS)
Measure Description
Percentage of children 2 years of age who had four diphtheria, tetanus and acellular pertussis (DTaP); three polio (IPV); one measles, mumps and rubella (MMR); three haemophilus influenza type B (HiB); three hepatitis B (HepB), one chicken pox (VZV); four pneumococcal conjugate (PCV); one hepatitis A (HepA); two or three rotavirus (RV); and two influenza (flu) vaccines by their second birthday.
Compliance:
Children meet criteria when they receive all required vaccines listed below on or before their second birthday.
Note: Compliance may be met through any listed criterion for each vaccine, including documented vaccine or antigen doses, history of illness, or history of vaccine-related anaphylaxis or encephalitis.

Notes
- Do not count DTaP, IPV, HiB, PCV, or RV vaccines given before 42 days after birth.
- MMR, VZV, and HepA must be given on or between the child’s first and second birthdays.
- For MMR, children must have a history of measles, mumps, and rubella to meet criteria based on childhood illness.
- For influenza, one of the two vaccines may be a live attenuated influenza vaccine (LAIV) if given on the child’s second birthday.
- For HepB, the first dose may be a newborn HepB vaccine.
- Vaccines given even one day after the second birthday are not valid for HEDIS® reporting.
Exclusions
- Members receiving hospice care.
- Members who die during the measurement period.
- Members with contraindication to a childhood vaccine on or before their second birthday.
Best Practices
- Schedule office visits to align with required immunizations.
- Use EMR alerts to identify when immunizations are due.
- Review each patient’s immunization schedule and status during office visits.
- Educate parents on how immunizations help protect their child, family, and community from vaccine-preventable diseases.
- Send reminders by email, mail, or text to help improve immunization rates.
- Request prior immunization records for new patients.
- If a child should not receive an immunization due to childhood illness, document the illness and date in the medical record.
- Submit claims promptly.
- Report all immunizations to the state immunization registry.
