Child and Adolescent Well-Care Visits (WCV)

Measure Description

Members 3-21 years of age who had at least one comprehensive well-care visit with a PCP or an OB/GYN practitioner in the measurement year.

Notes

  • This measure applies to members ages 3-21 years as of December 31st of the measurement year.
  • The well-care visit must occur with a PCP or an OB/GYN practitioner. 
    • Telehealth visits do not count as a well-care visit.

Exclusions

  • Members in hospice during the measurement year.
  • Member who died during the measurement year.

Best Practices

  • Encourage parents to establish care with a PCP and educate parents on the importance of preventive care visits.
  • Educate patients/parents on the importance of screening and counseling on healthcare topics done at each well-care visit.
  • Make outreach calls and/or send letters to advise members of the need for a visit.
  • Remind parents of the upcoming appointment for their child or adolescent by making calls, sending text messages or utilizing patient portal messaging.
  • Promote signing up for patient portal if facility provides it (i.e., MyChart).
  • Consider completing Well-Care visit during sick visits.
  • Incorporate well-care visits with sports physicals.
  • Schedule subsequent well-child visits at completion of current visit. Consider implementing an appointment scheduler to come to the patient room.
  • Consider hosting a Well-Care visit blitz after hours.
