Have you received a letter about Comprehensive Medication Management Services?

Comprehensive Medication Management (CMM) services are optional services and will be available to eligible members. Eligible members will receive letters in the mail notifying them of their eligibility to receive CMM services.  Eligible members who choose to receive CMM services must be seen by an Outcomes enrolled pharmacist in order to receive services. We encourage you to make the most of the CMM services offered.

What is CMM?

CMM services are pharmacist-led services that evaluate an eligible member’s medication regimen to ensure they are appropriate, effective, and safe.

As part of your appointment, you and your pharmacist may:

  • Make a medication treatment plan
  • Evaluate how well your medications are working for you
  • Discuss side effects you are experiencing
  • Provide extra training you may need to use your medications
  • Administer medications you may need help taking
  • Order or perform needed lab tests
  • Coordinate with your medication prescriber to address any medication changes  

Who is eligible for CMM?

CMM services are available to eligible members on individual or fully insured group health plans that receive a letter in the mail because the met one of the following criteria listed below:

  • The member is taking five or more chronic medications
  • The member was admitted to a hospital with one of the following diagnoses:
    • Congestive heart failure
    • Pneumonia
    • Myocardial infarction
    • Mood disorder
    • Chronic obstructive pulmonary disorder (COPD)
  • The member has an active diagnosis of comorbid diabetes and one of the following:
    • Hypertension
    • Hyperlipemia

Where can I get CMM services?

The pharmacist providing CMM services must be enrolled with Outcomes in order for you to receive CMM services.  You can locate an enrolled pharmacist that can provide CMM services by reviewing the participating provider list below or calling the number on the back of your member ID card.
