What Is Care Connect?

As part of its care management program, BCBSND connects care coordinators – registered nurses and licensed social workers – to eligible members to support their well-being. Enrollment is voluntary and at no additional cost to the member.

BCBSND will invite you to download the app if you’re eligible. You’ll receive an access code that you’ll use to complete your registration.

The BCBSND Care Connect app complements the online BCBSND Member Portal for qualified members. The Member Portal is a helpful platform to understand your health plan and to find a doctor in your network, things that can help everyone make informed health decisions. The BCBSND Care Connect app will feel tailored specifically to your health needs, providing you with the resources to help make it easier to manage complicated conditions and adopt healthy behaviors.

Wellframe is an independent company providing the technology that enables the BCBSND Care Connect app. The company specializes in technology to enable person-to-person connections and improves the care management experience.

The access code you receive is required to complete your registration. It will be a series of letters and/or numbers provided by BCBSND that you’ll use to register for the BCBSND Care Connect app.

You’ll be prompted to create an account during the BCBSND Care Connect app registration process.

If you’re having trouble downloading the app, check to see that you have enough memory available, are connected to WiFi, and have a mobile operating system that’s compatible with the app.

The app is compatible with Apple devices running iOS 14 and newer, including iPhone^®´, iPad^®´, and iPod touch^®´. It’s also compatible with smartphones and tablets running on Android^TM 8 and newer, which run on brands such as Pixel, LG, Moto X, Nexus, HTC, and Sony.

The BCBSND Care Connect app uses minimal data to work properly on your smartphone or tablet. It’s unlikely to cause you to go over your data limit, especially if you’re using the app over a WiFi network.

The app only uses general user data that is needed to function, which is saved securely on a cloud platform that is managed by a trusted third party.

BCBSND account managers will reach out to customers who have been included with the initial rollout of this new digital tool.

For companies that haven’t been included, there will be a much larger rollout with additional notifications to let you know when it’s available to other members.
