What is care management?

Care management connects you with a specialist who is dedicated to helping you understand your health and your care plan. They work with you, your doctors, and your support system to make sure you have what you need to stay on track.

When care management can help the most

Care management is helpful if you or a family member:

  • Have a chronic condition, such as diabetes or heart disease

  • Are dealing with a new or serious diagnosis

  • Have multiple doctor visits or hospital stays

  • Need help understanding treatment options

  • Have a high-risk pregnancy

  • Want extra support managing your health

Meet your care managers

Our care managers are nurses, social workers, and mental health specialists that have experience with many health needs.

They are here to listen to your concerns, answer your questions and help guide your care. Think of them as someone you can trust to help with your health. And they’re available at no extra cost.

Care managers may contact you if we think extra support could help. But you don’t have to take part. This can happen after: 

  • A hospital stay

  • Frequent emergency room visits 

  • A new diagnosis

  • Changes to your care
