The bracketed sections of the Certificates of Insurance indicate a variable benefit that will be finalized by each employer during the application process. BCBSND's electronic display of these Certificates of Insurance does not constitute a contract between any employer and BCBSND or any employee and BCBSND. To obtain group health insurance coverage from BCBSND, an employer's application and premium must be accepted by BCBSND and employees must satisfy certain eligibility requirements. Upon acceptance BCBSND will issue a paper Certificate of Insurance setting forth the benefits available under the group health insurance coverage.
